Register as an activity provider

Thank you for expressing an interest in becoming a Get Hooked on Life activity provider. We are excited about putting your business and the inspiring activities you offer in front of 75,000 young people aged eight to eighteen in Sheffield. In order to become an activity provider, there are three straightforward steps to follow:

1 . Become quality assured

The Get Hooked on Life quality standard, endorsed by the local authority, verifies that you offer safe, high quality provision to Sheffield's young people.

Before you arrange your registration appointment, you will need to make sure you can show us the documentation listed below. This should only take you a few minutes to gather.

  • A digital copy of your public liability insurance cover sheet.
    Our provider's minimum indemnity must be £2 million.
  • A digital copy of employers liability insurance cover sheet (if you are an employer).
    Our provider's minimum indemnity is £2 million.
  • Your most recent child protection policy.
  • A named child protection officer.
  • A full staff list of employees who have, or are likely to have, access to young people in an activity session or through the website.
    We also need their enhanced disclosure numbers, issue dates and details of issuer. Please note, with Get Hooked on Life an enhanced disclosure is only acceptable for three years. If you find your enhanced disclosure has run beyond three years we will shortly be offering our own enhanced disclosure service.
  • Your acceptable use policy.
    If you do not yet have an acceptable use policy you can customise our acceptable use policy matrix to your own requirements free of charge.

Now you have collated this information, please complete this initial two minute provider registration form and we will be in touch with you shortly to make an appointment. You may wish to print off a downloadable checklist of the above criteria to help you plan for this meeting.

2. Create your profile

Usually we aim to contact you by telephone within five working days. By then, you will need to have decided which of the three options, shown in the list below, you would like to choose to create your online presence. This is likely to depend on how comfortable you or a team member feels with this level of technology.

3. Update your profile regularly

Which ever option you have chosen, you will now be ready to go. Please be sure to keep your profile up to date with any activity, staff or organisational changes!

Your page will look much fresher and more exciting if you are uploading new information and photographs on a regular basis. Click on this case study to see what we mean. The more interesting your site is, the longer they will spend looking at it.
